What the Role Looks Like Day-to-Day As an Addiction Counselor at Kolmac, you’ll be at the center of patient care — building strong therapeutic relationships and delivering high-quality, evidence-based treatment in our Hagerstown outpatient clinic. You’ll hold a blend of individual sessions, group facilitation, and case management, with approximately 55% of your time in direct patient care. The rest is split across documentation, treatment team collaboration, and care coordination.
Your core clinical responsibilities: Build a genuine, individualized therapeutic alliance with patients that adapts to their evolving clinical needs throughout treatment
Facilitate addiction treatment groups across OTP levels of care
Provide substance use disorder evaluations using DSM-5 and ASAM level-of-care criteria
Prepare and present addiction/recovery education to patients — embody the spirit of a teacher inside and outside of sessions
Conduct case management sessions: assessments, collaborative treatment goals, and discharge planning
Apply evidence-based modalities including Motivational Interviewing, CBT, and trauma-informed care
Work with the treatment team to address co-occurring psychiatric needs
Coordinate care across support services — family members, referral sources, and community providers
Provide evaluations and maintain timely, well-written documentation meeting state and CARF standards
Track clinical outcomes: patient retention, attendance, program completion, and treatment goal attainment
Participate in a shared after-hours crisis on-call rotation (one week several times per year — details discussed in interview)
Attend and contribute actively to treatment team meetings and professional development
A note on schedule: This role operates Monday–Friday, 5:30AM–1:30PM, 6:00AM - 2:00PM, and 7:00AM- 3:00PM reflecting the needs of our patient population who require morning services before work or other daily obligations. Your afternoons are your own. If you’re energized by a structured morning rhythm, this schedule can be a real advantage.
